Day Habilitation
Day Habilitation – Providing Daily Opportunities to Community Access
Our unique Day Habilitation Service creates a safe place to enhance social skills, develop new abilities and have fun. Every day is meaningful for our Members and staff!
Transitions Centers Day Habilitation promotes community involvement. Our Members advocate for their activities and choose those where they are active, such as doing a variety of leisure, volunteer and physical activities. TCI staff work with our Members to develop the activities that they advocate for, and they work to make those activities a reality.
We are the only program on Cape Cod to provide specialized services for those members who communicate through American Sign Language (ASL).
Individuals who take part in our Day Habilitation program must be able to work within a 1:7 staff to Member ratio. They must also enjoy community involvement, be active, socialize and have fun.
Our Day Habilitation Service assists with:
- Making healthier choices with food, exercise and diversity training
- Independent living skills with cooking and cleaning
- Increased socialization and social skills classes
- Promoting Self Advocacy
- Involvement in the community through volunteer opportunities that increase community involvement skills
Our staff works closely with our Nursing support staff and contracted therapists to provide the following services to members who require it:
- Occupational Therapy
- Physical Therapy
- Speech Therapy
- Behavior Development
- Social Skills Therapy
Transitions Centers has a sensory room available. Members are trained on what strategies and tools they can utilize to help with self-regulation and an exercise room to be able to work on stamina and to develop good habits of daily exercise. In addition, assisting in developing positive coping strategies.
Entrance Criteria
To be eligible for Transitions Centers Day Habilitation Services, a potential Member must:
- Be eligible for MassHealth Services
- Set up a referral meeting with the Day Habilitation Manager.
- Must be willing to actively participate in community-based activities.
- Can work within a 1:7 staff: Member ratio.
- Complete the TCI Referral Packet and include all necessary documentation requested in the packet.
- Provide any additional information requested by the Interdisciplinary Team.
