With the support of Latham Centers, our parent company, Transitions Centers was created on June 13, 2008, with just four Members and two staff. That was fifteen years ago this month! Transitions Centers has changed quite a bit since our first day that we opened. The agency has always been in South Yarmouth, moving from a rented space on Commercial Street to purchasing our headquarters on Route 28. We’ve also grown and now serve 100 Members from Falmouth to Wellfleet. Some Members have been with us since the beginning, and we’ve added many others through the years. We’ve added services along the way, our first service being Day Habilitation, then adding our Supported Employment Service, and then adding our Individual Support Service. We’ve had the pleasure of employing many talented staff and despite the changes in faces, the quality of our services has always been high and the focus of what we do has always been consistent: Our goal is to improve the lives of neuro-diverse people and give them the skills to make them as independent as possible in the real world. Our focus is quite simple, to work with each individual so they have increased competence and confidence in the community. There have been good times as well as challenges in our past fifteen year and we expect more of these over the next fifteen years. Everyone at Transitions Centers feels proud of what has been accomplished so far, but we all look forward to greater achievements in the future!
